Medicare Coordination – Mental Health & Substance Use Services with CDHP

If you have Medicare Parts A and B:

No precertification is needed if Medicare covers your mental health or substance use service.

Precertification is required if Medicare doesn’t cover the service or hospital days are exhausted.

When Medicare is the primary payor, the CDHP limits payment to the difference between the Plan’s liability and Medicare’s payment. If Medicare pays the same or more than the Plan’s liability, no additional payment is made.

Preauthorization Requirements – Mental Health & Substance Use Care

Preauthorization helps confirm that your care is medically necessary and covered. Call 855-511-1893 for preauthorization before starting:

When required:

  • Inpatient Hospital Care

  • Residential Treatment Center admissions

  • Intensive Outpatient Programs (IOP)

  • Partial Hospitalization

  • Psychological Testing

If Medicare is your primary coverage:

  • Preauthorize if Medicare does not cover the service, or

  • You’ve exhausted Medicare hospital benefits and choose not to use lifetime reserve days.

No precertification is required for services outside the U.S.

Yes — Preauthorization is required for inpatient stays, RTC admissions, and certain outpatient programs. Please call 855-511-1893 for more information.
Yes — In-network telemental and virtual visits are covered. The patient will be responsible for 10% of the Plan allowance.
Yes — your PCA covers 100% of eligible services until funds are exhausted, then standard cost-sharing applies.
A $500 benefit reduction will apply and coverage may be denied.
