With the NALC High Option Plan, covered routine checkups, screenings, vaccines, women’s health services, and more are all paid at 100% when you use in-network providers. What You Need to Know About Your High Option Preventive Care

Preventive care is at the core of the NALC High Option Plan. By focusing on early detection and healthy habits, members can protect their long-term health and avoid costly treatments later.

In-Network Preventive Care — $0 Cost

  • 100% coverage for preventive services

  • No deductible

  • No copays

  • No effect on your out-of-pocket maximum

Out-of-Network Preventive Care

  • 35% coinsurance of the Plan allowance

  • You may also owe the difference between what the provider bills and the Plan allowance

  • Save the most by staying in-network

Preventive Care for Adults

Your health matters at every stage of life. With the NALC High Option Plan, members receive full coverage for essential preventive services.

What’s Included

  • Annual routine physical exam (age 22+)

  • Routine colonoscopy & sigmoidoscopy as recommended by the USPSTF.

  • USPSTF A & B screenings: cancer, osteoporosis, depression, diabetes, high blood pressure, cholesterol, HIV, colorectal cancer

  • Annual A1C test (age 18+)

  • One-on-one counseling for reducing health risks

  • Adult immunizations as recommended by the CDC

Women’s preventive care

Pap smears, STI counseling, contraceptive methods, newborn prophylaxis, and domestic violence screening

Routine mammograms:

  • Ages 35–39: one screening during this period

  • Age 40+: one each year

  • Breast tomosynthesis (3d mammogram) when performed in conjunction with a routine screening mammography.

Preventive Care for Children and Young Adults

Healthy beginnings lead to strong futures. The NALC High Option Plan covers preventive services for children and young adults at no cost when you use in-network providers.

What’s Included:

  • Newborn exam (with family enrollment)

  • Routine well-child visits through age 2

  • Annual physicals ages 3–21 (includes school, camp, and sports — not covered at CVS MinuteClinic®)

  • Annual A1C test starting at age 18

  • Immunizations as recommended by the CDC

Reward Opportunity: Earn $50 in health savings rewards when your child completes six well-child visits before 15 months.

Biometric Screenings and Wellness Rewards

Take charge of your health with a free annual biometric screening — fully covered for members when rendered by an in-network provider.

Includes

  • BMI calculation

  • Waist circumference

  • Total cholesterol

  • Blood pressure check

  • Fasting blood sugar

Wellness Rewards:

  • $50 for your annual biometric screening

  • $10 for your annual flu vaccine

  • $10 for your annual pneumococcal vaccine

The NALC High Option Plan helps our members manage weight and nutrition with personalized programs and professional guidance.

Covered Services

  • Intensive nutrition and behavioral weight-loss counseling

  • Family-centered programs for obesity prevention

  • Classes and therapy from a registered nurse or dietitian

  • Virtual nutrition counseling through the NALC HBP Telehealth app

  • Real Appeal® Program: free online group and 1-on-1 coaching, fitness-on-demand tools, and nutrition support

Additional Coverage Notes:

  • FDA-approved anti-obesity medications when prescribed in conjunction with one of the Plan's weight loss programs.

  • Bariatric or metabolic surgery is covered when medically necessary

What’s Not Covered Under Preventive Care

To avoid unexpected costs, here are services that do not count as preventive care:

  • Routine lab tests not listed under the preventive benefits section of our official Plan brochure.

  • Travel or work-related vaccines and medications

  • Employment, insurance, or travel physicals

  • Services not recommended by USPSTF, CDC, or HHS guidelines
